You must follow the following simple steps to add a subscription link to your video.
- Log in to your YouTube account
- Click on your video manager
- Click on one of the videos that you have uploaded
- Click on the Add annotations dropdown as shown in the screenshot below

- For example, choose Speech Bubble.
- Write "Click here to subscribe to my channel"
- Choose the font size
- Choose the color
- Choose the start time to show the subscription box
- Choose the end time.
- Click on link checkbox
- Click on subscribe
- Enter your YouTube username
- After you are done with all above steps, click on Save
- Click on Apply changes
